Occupational Therapist (Home Based Primary Care)

This position will be for the HBPC Patient Aligned Care Team (PACT) which serves Veterans in 23 counties in Northeast Ohio. HBPC PACT provides longitudinal primary and ancillary services to homebound veterans with complex medical and psychosocial conditions. The Occupational Therapist visits at the patient's primary residence. The therapist completes evaluations to determine the patient's function and safety in their home and community. Evaluations of the patient include, but are not limited to, assessment of:
range of motion, strength, fall risk, pain, pressure reduction, activities of daily living and instrumental activities of daily living. The patient's home is also evaluated for safety and environmental hazards. The therapist orders or suggests equipment to improve veterans' safety and function. The therapist will conduct training with equipment as indicated. The therapist also will recommend and implement a rehabilitation program if indicated. The therapist collaborates with the HBPC interdisciplinary team of medical professionals. This position will also require communicating with agencies external to the VA to coordinate patient care. United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of a degree program in occupational therapy approved by the Accreditation Council for Occupational Therapy Education (ACOTE) to include an internship (supervised fieldwork experience required by the educational institution). Possession of written documentation that the individual has passed the entry-level certification examination for OTs which is administered by the National Board for Certification in Occupational Therapy (NBCOT). A full, current, and unrestricted state license, certification, or registration or trademark to practice occupational therapy in a State, Territory, a Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia. Must possess a valid driver's license. Non-licensed OT's, who otherwise meet the basic requirements, may be given a temporary appointment as a graduate OT not-to-exceed two years. Non-licensed individuals appointed as a temporary graduate OT may not be appointed as an OT without being licensed. Preferred

In addition to meeting the basic requirements, completion of one year of progressively complex experience and a broader scope of experience equivalent to the next lower grade directly related to the position being filled, or 3 years of progressively higher level graduate education leading to a degree in occupational therapy or a directly related field. OTs at this grade level must meet state regulatory requirements for assignment at this grade level, and are considered to be at the full performance level. Staff OTs at this level practice independently and are responsible for performing occupational therapy assessment and treatment interventions. Staff OTs demonstrate a full professional understanding of the theories of treatment in occupational therapy and their proper application through the use of activity analysis, behavioral intervention, frame of references, and therapeutic procedures. Physically be able to perform heavy lifting, 45 pounds and over. Must have the use of:
fingers, both hands required, both legs required (to include one prosthetic), vision as needed for driver's license, hearing (aid permitted), and mental and emotional stability. Employee must be able to perform reaching above shoulder, walking (up to 8 hrs.), standing (up to 8 hrs.). Must be able to negotiate multiple flights of steps while carrying a treatment bag.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
