Skypoint Recovery is looking for Community Case Managers to work with our clients in assisting them in an array of services.

EDUCATION/EXPERIENCE/LICENSE/CERTIFICATION Required:

The Case Manager will be responsible for providing comprehensive case management services to clients, focusing on assessing their current needs, referring to appropriate services, and offering ongoing support.
Responsibilities:
Needs Assessment:
Conduct thorough assessments of clients' current needs related to substance use, mental health, housing, employment, and other social determinants of health.
Utilize standardized assessment tools and clinical interviews to gather information and develop a comprehensive understanding of clients' strengths and challenges.
Referral:
Connect clients with appropriate resources and services based on their identified needs, including substance use treatment programs, mental health counseling, medical care, housing assistance, vocational support, and peer support groups.
Advocate for clients to access necessary services and navigate the healthcare and social service systems.
Build rapport with outside agencies and conduct self in a professional manner.
Monitoring and Support:
Provide ongoing support and advocacy to clients throughout their recovery journey.
Monitor progress towards their goals, identify barriers to success, and collaborate with clients to develop strategies for overcoming challenges.
Offer crisis intervention and support during times of heightened need.
Collaboration:
Work closely with other members of the treatment team, including therapists, case managers, nurses, techs and community partners, to ensure coordinated and comprehensive care.
Participate in multidisciplinary team meetings and case conferences to discuss client progress, coordinate services, and develop integrated treatment plans.
Documentation:
Maintain accurate and up-to-date records of client assessments, progress notes, service referrals, and treatment plans in accordance with agency policies and regulatory requirements.
Complete necessary documentation in a timely and thorough manner to ensure continuity of care and compliance with funding requirements.
Education and Prevention:
Provide education and outreach to individuals and communities about substance use disorders, harm reduction strategies, treatment options, and available resources.
Participate in community events, workshops, and training to raise awareness and promote prevention efforts.
Crisis Intervention:
Provide crisis intervention and support to clients during times of heightened need, including assisting with emergency housing, accessing crisis services, and providing emotional support and de-escalation.
