Community Health Worker I Reports to:
Primary Prevention and At-Risk Populations Manager Summary of Position:
Serves as a liaison between the community and the health care, government, and social services systems to provide clients with the psychosocial support needed to cope with chronic, acute, or terminal illnesses and reduce infant and maternal mortality.
Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs.
Advocates for clients or patients to resolve crises.
Facilitates access to resources and services to improve service delivery quality and cultural competence.
Minimum
Qualifications:
1.
Formal Education Required:
a.
High School Diploma or equivalent 2.
Experience & Training Required:
a.
Completion of approved Certified Community Health Worker Training Program b.
CPR 3.
Other Skills, Competencies and
Qualifications:
a.
Certification through Ohio Board of Nursing b.
Basic computer skills c.
Must pass background check and drug test d.
Knowledge of basic medical terminology e.
Ability to work with diverse communities and different races, cultures and classes f.
Ability and willingness to provide emotional support, encouragement and motivation to clients g.
Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity h.
Ability to obtain and record vital signs, and recognize changes in health status i.
Second language skill is a plus 4.
Level of Physical Demands:
a.
Light:
Exerts up to 20 pounds of force occasionally and/or up to ten pounds of force frequently, and /or a negligible amount of force continuously.
